HOW TO BE THE WESTPEAK FAMILY
STEP 1
Complete referral forms that are located online or go to your nearest Westpeak SRS for a hardcopy form.
STEP 2
The SRS manager will arrange a face-to-face meeting as well as a tour of the SRS.
STEP 3
The manager will contact supports and determine how much care you require.
STEP 4
Manager will assess the skills and qualifications of the staff to determine if the SRS can support your individual needs.
STEP 5
If the SRS is able to support you; sit back, relax and welcome to the Westpeak family.
